Add User/Team notifications actions for Form Capture

The current construct for form capture does not provide a Design > Action option.   If you want notifications to go out to users and teams with content received from the form post, you have to create it yourself.  Additionally, since it requires that the information reside either on lead or contact, it creates various pitfalls (see http://support.clickdimensions.com/entries/22120105-Provide-option-with-Form-Capture-or-Web-Content-Form-to-always-generate-new-lead).

It would be nice if we could utilize the email notification actions in some fashion, because it would be more rodust than having dependence on values showing up on a record (when we may not want to override in CRM).
