Subscription Management Page - Allow Name to be Collected

On the Subscription Management page you are very limited in the information you can collect, we would like to also be able to add the first name and last name fields and possibly address and company.  If we have new contacts that are subscribing to our website we would like a little more information than just email.

    We are not ecommerce, so we need to be able to capture name (and ideally company) in order for our sales team to be able to use the leads from our email campaigns.

    This is huge! Our business rules say that only a Contact record with a first name, last name and some form of contact info (email, for example) are allowed. We have to piece together a two part email subscription to capture the first and last name.
